School Based Rehabilitation Services
Therapy where children spend their day
Five Counties Children’s Centre provides in-school rehabilitation services to enable the student’s attendance and participation in publicly funded educational settings. School-Based Rehabilitation Services include Occupational Therapy (OT), Physiotherapy (PT) and Speech-Language Pathology (SLP). Referrals are either made internally through Five Counties’ staff or by School Board staff for those children identified at school.

A seamless approach to getting the care your child needs
Coordinated Services Planning — what is it?
This program develops a coordinated plan of care with the family and all service providers. Meetings are held regularly with a Service Planning Coordinator. Coordinated Service Planning focuses on all areas of a child’s development: fun, family, friends, function, fitness, and future.
Who is eligible?
Children and youth with multiple and/or complex special needs; children and youth under the age of 18, and young people between the ages of 18-21 who remain in school; families who reside in Peterborough, City of Kawartha Lakes, and Haliburton County (families in Northumberland County can access Coordinated Services Planning through the Quinte Children’s Treatment Centre).
How to connect?
Families can self-refer through the SmartStart Hub’s Request Services portal. Referrals are also accepted from physicians, schools, and community agencies (with consent of the family).
