School Based Rehabilitation Services
Five Counties Children’s Centre provides in-school rehabilitation services to enable the student’s attendance and participation in publicly funded educational settings.
What is included
Three services, delivered at school
Referrals
Referrals are either made internally through Five Counties’ staff or by School Board staff for those children identified at school (the online School Referral Form). Parents of children not currently involved with Five Counties Children’s Centre are encouraged to speak to their child’s teacher if they have questions about whether their child may be eligible for the service.
Children who are home schooled or attend private schools
Families whose children are in Grade 1 and higher grades and attending a private school or being home-schooled can access treatment services through the Ontario Health atHome’s School Health Support Services. For children who are in JK/SK and are being home-schooled or attending a private school, they can continue to receive treatment services through Five Counties.
For school staff
The School Referral Form
School Board staff refer children identified at school through the online School Referral Form. In the redesign the form becomes an accessible CMS form with confirmation, routing to the right site, and a printable alternative — no PDF round-trip.
